Application Process

4-Year Baccalaureate Nursing Program

1. Download the Application Checklist to help organize your application materials
2. Download the Acceptance Criteria Form to see how your application will be scored
3. Complete the ASC Application for admission to ASC
4. Complete the 4-Year BSN Nursing Application
5. Submit the completed nursing application to the Nursing Dept.
6. Submit official college transcripts to the address below

RN-BSN Nursing Program

1. Complete the online application for admission to ASC
2. Complete a RN-BSN nursing application
3. Submit the completed nursing application to the Nursing Dept.
4. Submit official college transcripts to the address below
5. Submit a copy of your RN license to the address below
6. Schedule an advising session by calling Carol Otto at 719-587-8171

Background Check Instructions

1. Download the Background Check Instructions
2. Contact Carol Otto at 719-587-8171 to request a fingerprint/drug-test packet

Immunization Instructions (Required prior to clinical but not BSN admission)

1. Download the Immunization Requirement Letter
2. Download the Immunization Requirements
3. Complete the Student Immunization Certification

Mail the nursing application, transcripts, and RN license (if applicable) to:

Application Deadlines:
