Personal Service Forms

If you will be bringing someone in as an Independent Contractor the following forms must be filled out by them and sent in with your purchase requisition.

• W-9
• CERTIFICATION AND AFFIDAVIT
• EXHIBIT IC

If it is for any personal services over $5,000 please also attach the Personal Service Waiver form.
