For your convenience, we are providing the following checklist in a printable format to assist you with the application process.

Additional Forms

The Admission/Registration Form is used for students who wish to take classes (up to 12 credit hours) without first being admitted to a graduate program or students that are registering for workshops. Students who use this form are classified as non-degree seeking "Graduate Special" students and are not eligible for financial aid.

The Special Re-Admission Form is for students that have been admitted to a graduate program, but did not take classes one or more semesters. This form must be filled out and returned to the Graduate School before the student is able to register again.

The Intent to Graduate form must be filled out and returned to the Graduate School office by the deadline listed on the form. Students graduating in the Summer may choose to walk in Spring or Fall as there is no Summer commencement ceremony. Please contact the Graduate School Office for details.

The International Graduate Student Application form is for anyone who wishes to apply for admission to the Adams State Graduate School and is not a US citizen or permanent resident of the US.

This form is used to initiate the approval of an independent study course. Approval is needed by the Instructor, the Department Chiar and the Director of the Graduate School
