Help Us Keep Our Campus Safe and Well
It is all of our responsibility to make sure our campus community is safe and well. There are times that you may feel like you want someone to know about something but aren’t sure who to tell or who you can talk to. The reports below are meant to take out the guesswork. Each of these forms is automatically routed to the office responsible for responding to it.
Note: Before filing a report, know that we encourage issues to be resolved at the most informal level possible.
Connecting Students with Resources
Person of Concern: Use this form to report a concern about a student or employee – change in behavior, change in attitude, concern for their health and wellbeing.
Academic SOS: This form should be used to share information about academic concerns for a student that might inhibit their ability to be successful.
Incident Reporting
Housing Incident: Use this form to submit information about violations of housing policies.
Student Code of Conduct: Use this form to submit information about violations of the student Code of Conduct.
Title IX or Workplace Harassment: Use this form to submit information about any type of workplace harassment (sexual in nature or otherwise) or any type of sexual misconduct including assault, dating violence, stalking, domestic violence, etc.
Student Complaints and Grievances
Student Complaint/Grievance: Use this form if you are a student and would like to make a complaint to the Office of Student Affairs about a campus or classroom issue.
