Immunization Requirement Information
Students participating in Adams State on-campus classes and activities must fulfill state immunization requirements prior to attending. These requirements consist of the MMR (Measles, Mumps & Rubella) and the Meningococcal vaccinations. Students will be required to provide proof of vaccination or submit a waiver form.
MMR (Measles, Mumps & Rubella) Vaccination is required for all students who will be attending on-campus classes; living in campus housing or participating in any on-campus activities. Proof of immunity can include one of the following:
- Official documentation showing both doses of the two-part vaccine have been administered
- A titer test that shows immunity
- Born before January 1, 1957
If a student wishes to waive the vaccination requirement due to medical or person reasons, a waiver form must be completed and submitted to the University. The waiver form is accessible on the Colorado Department of Public Health and Environment Website If an outbreak of these diseases was to occur, and the student is not vaccinated or immune, they may be quarantined or not be allowed on campus until the outbreak was resolved.
Meningitis Vaccination (Meningococcal ACWY) is required for students living in campus housing; however, it is recommended that all students be immunized. Proof of immunity includes official documentation showing the last vaccination of Meningococcal ACWY has been received within five (5) years from the start of the semester attending. If a student wishes to waive the vaccination requirement due to medical or person reasons, a waiver form must be completed and submitted to the University.
