Are you symptomatic: fever, dry cough, difficult breathing?

• If Yes to symptomatic, Have you been tested for COVID-19?
  • If Yes to tested, Are you positive for COVID-19 or have been quarantined by a health officer?
    • If Yes to positive, Self isolate and take care of yourself. Work with HR and your supervisor on your estimated return to work date.
      • Have you been symptom free and are you ready to return to work?
        • If Yes to symptom free, Please fill out the State of Colorado Employee Self Certification Form for COVID-19 like Symptoms.
    • If No to positive, Are you well enough to work?
      • If No to well to work, Self isolate and take care of yourself. Work with HR and your supervisor on your estimated return to work date.
        • Have you been symptom free and are you ready to return to work?
          • If Yes to symptom free, Please fill out the State of Colorado Employee Self Certification Form for COVID-19 like Symptoms.
  • If No to tested, Please call 877-462-2911 CO Help or 719-589-2511 option 9 to conduct a risk assessment.
• If No to symptomatic, Have you been in close contact with someone who is symptomatic or traveled to an at risk area?
  • If Yes to close contact, Can your job be performed remotely?
    • If Yes to remote, Work with your supervisor and HR on your remote work agreement, then work from home.
    • If No to remote, Work with your supervisor to assess the impact of your duties if you are our for an extended period of time.
  • If No to close contact, Have you been told to self isolate by a health official or executive leadership?
    • If Yes to self isolate, Can your job be performed remotely?
      • If Yes to remote, Work with your supervisor and HR on your remote work agreement, then work from home.
      • If No to remote, Work with your supervisor to assess the impact of your duties if you are our for an extended period of time.
    • If No to self isolate, Are you immune compromised or in a high risk group?
      • If Yes to high risk, Can your job be performed remotely?
        • If Yes, Work with your supervisor and HR on your remote work agreement, then work from home.
        • If No, Work with your supervisor to assess the impact of your duties if you are our for an extended period of time.
      • If No to high risk, Do you have dependents at home who require supervision?
        • If Yes to dependents, Can your job be performed remotely?
          • If Yes to remote, Work with your supervisor and HR on your remote work agreement, then work from home.
          • If No to remote, Work with your supervisor to assess the impact of your duties if you are our for an extended period of time.
        • If No to dependents, Maintain normal work schedule & practice social distancing.
