Replacement diplomas are issued by the Office of the Registrar only upon request. A written request should be mailed to:

The request should include the student’s full, legal name, the name under which
the student attended, the name as s/he would like it to appear on the
replacement diploma, the requestor’s contact information, date of birth, year of
graduation, program of study and the address where the diploma should be
mailed. A check for $30 made payable to Adams State University should accompany
the request. You may instead include credit card information as follows: name as
it appears on the card, credit card number, expiration date and security code
