Application forms for initial licensure and added endorsement are found on the Colorado Department of Education (CDE) web site: cde.state.co.us
- Download the verification page that requires the “Dean of Education or Certification Officer” signature.
- Complete the top portion and then email, fax or mail this form to the Adams State Teacher Education Department.
- Once this form has been completed and signed by the licensure officer, it will be emailed to you to upload with your application.
- The original verification form will be mailed to you for your records.
You are responsible for ordering your official transcripts.
